Commissioning Process
A Seamless Referral Pathway for Your Pupils
We partner with schools to provide a clear, collaborative transition for pupils requiring specialised, individualised support beyond the mainstream setting. Our commissioning process is designed for clarity and efficiency, ensuring we build on the excellent foundations you have established.
Our Collaborative Six-Step Process:
1. Professional Referral
Initiate the process by contacting our dedicated team. We’ll gather initial details about the pupil and your school’s concerns, ensuring we are the right fit to meet their complex needs.
2. Joint Consultation
We meet with you, key school staff, and often the local authority, to discuss the pupil’s EHCP, current provision, specific challenges, and desired outcomes. This ensures a complete handover of insight.
3. Comprehensive Learner Profile
Building on existing reports and your expert knowledge, we conduct holistic assessments to create a dynamic learner profile. This details cognitive, communication, sensory, behavioural, and social-emotional needs, forming the blueprint
for their programme.
4. Designing an Individualised Learning & Support Programme
Our specialists draw upon all information to design a bespoke curriculum and care plan. This aligns with EHCP outcomes and focuses on life skills, engagement, and personal development, ready for your review.
5. Managed Transition & Placement Start
We plan an appropriate, sensitive transition to minimise anxiety. Your pupil begins their tailored programme with us, supported by a dedicated key worker.
6. Ongoing Reviews, Reporting & Partnership
We maintain strong communication with you and the local authority through termly review meetings and regular progress reports. This ensures the placement continues to meet evolving needs and contributes to annual EHCP reviews.
